HMO's also have "capitations"- your PCP is capitated to different sites for specific tests... Dr Smith's patients should only go Lakeview Hospital for Xrays. If Dr Smiths patient goes to Adams Hospital for Xrays, it will be rejected for "out of the capitated site".

when the policy goes in to effect, the best thing to do is to call them and talk to them about what they expect. If you have chronic care- see a therapist, go to pain management- anything that keeps you going back to the MD on a regular basis, then the insurance company will be able to help you out and let you know. Also, the staff at your doctors office will be able to answer some questions.
